Caregiver Support Program - Island Health Community Health Services (CHS)

Provides virtual support, monitoring, and education from home for patients with chronic conditions, acute illness, or recovery needs.
The Caregiver Support Program provides time-limited virtual emotional support for family caregivers with complex caregiving needs that cannot be fully supported through other Island Health or community services. Social workers support caregivers to set goals and build a personalized plan focused on emotional well-being, coping skills, and practical caregiving strategies.
The Caregiver Support Program may be appropriate for caregivers who require support with:
  • Brief one-to-one counselling
  • Caregiver stress, emotional strain, coping, or boundary-setting
  • Goal setting and development of a personalized caregiver support plan
  • Skill-building groups based on caregiver goals
  • Connection to additional caregiving resources and support options after program completion
Eligibility
  • Caregiver lives within Island Health region and is supporting a family member or friend with complex caregiving needs.
  • Caregiver needs support with coping, emotional well-being, boundaries, or practical caregiving skills
  • Eligibility depends on caregiver needs, the care recipient’s health conditions, existing supports, and care needs.
How to Access:

Access starts through the Community Access Centres using a central intake and assessment process. Services are available to clients throughout Island Health regions.
