Publicly subsidized Long-Term Care homes that provide 24-hour professional care and supervision to adults in a supportive and secure environment.
Publicly subsidized long-term care provides 24-hour professional care and supervision for adults with complex medical, physical, or cognitive care needs that can no longer be safely met at home or in assisted living. Long-term care is for people who require ongoing nursing care, daily personal support, and continuous supervision in a supportive and secure care home environment. Please note that eligibility is based on the urgency of the patient’s current care needs, not anticipated decline. Only patients who meet eligibility criteria are placed on a waitlist. If a patient is not eligible for publicly subsidized long-term care, other care options may be discussed.

Eligibility
  • Patient is a Canadian citizen, or have permanent resident status
  • Patient is a resident of British Columbia
  • Patient has complex medical, physical, or cognitive care needs that require 24-hour care and supervision
  • Patient’s needs can no longer be safely met at home or in assisted living, even with support
  • Patient has an assessed and urgent need for publicly subsidized long-term care
  • Patient meets admission criteria for one of Island Health’s long-term care homes
  • Eligibility depends on citizenship and residency requirements, diagnosis, care needs, urgency, safety, available supports, and whether the patient meets admission criteria for a publicly subsidized long-term care home.
How to Access:
  • Public can start this process by calling their local Community Access Centre (see below), or through an existing Island Health Case Manager. Healthcare Practitioners please review the Physician Information section below.
  • An Island Health Case Manager completes a standardized assessment to determine eligibility, care needs, urgency, safety, and whether long-term care is appropriate.
  • Before long-term care is considered, home and community care services may be explored to help the patient remain safely at home for as long as possible.
  • If the patient meets eligibility criteria, they may be placed on a waitlist based on assessed need, urgency, and availability.
  • If the patient is in hospital, the hospital care team will work with the Case Manager to support the patient while waiting for placement, if applicable.
  • When a space becomes available, the Case Manager will guide the patient and family through next steps, including timelines, what to bring, daily routines, visiting guidelines, and costs.
