Case Management for Home Care - Island Health Community Health Services (CHS)
Condition Specific Support and Home Health Care
Provided by Island Health
- Identifying care needs, service options, and any related costs
- Developing a care plan that is reviewed and adjusted as needs change
- Accessing Community Health Services, partner programs, and community supports
- Coordinating services such as Home Support, Adult Day Programs, Assisted Living, or Long-Term Care, when appropriate
- Working with health care providers, family, caregivers, and community supports to help meet care needs
- Supporting smoother transitions between hospital, home, assisted living, or long-term care
- Patient must be a Canadian citizen, or have permanent resident status
- Patient must be a resident of British Columbia
- Eligibility depends on health conditions, functional needs, existing supports, safety, and care needs.
