Intensive Case Management - Youth - Interior Health Area

Provides intensive case management services to high needs clients with substance use and mental health issues.
The Youth Intensive Case Management teams offer low barrier outreach services to meet you where you are at, without judgement. They will work with you to meet your immediate needs and connect you with supports and services to improve your health and meet your wellness goals. The Youth Substance Use Intensive Case Management Programs serves:
  • Youth and young adults 12-24 years old who have had difficulty accessing services
  • Those requiring intensive outreach and support who are unable to engage in traditional substance use and mental health services
  • Those who have significant substance use issues with possible mental health diagnosis and functional impairments (severe severity, and low complexity)
