Occupational and Physical Therapy - Community Health Services (CHS)

Information on the Community Health Services Occupational Therapy and Physiotherapy services, and how to access.
Home and community-based physiotherapy and occupational therapy services for those with complex physical, mobility, cognitive and/or environmental needs to optimize physical recovery and activities of daily living. The need for hospital visits is reduced with the assistance of physio and occupational therapists, as they provide physical rehabilitation, proactive care and associated supports.

CHS Physiotherapy (PT):
  • Patient has complex physical impairments & functional mobility needs
  • Patient's physical status is evaluated in order to develop individualized therapy programs
  • PT can optimize recovery from acute fracture, injury, illness, surgery, or exacerbation of chronic disease
  • PT can optimize mobilization in the patient's home (locomotion, gait, transfers, activity tolerance, stairs, etc.)

CHS Occupational Therapy (OT):
  • Patient has complex physical, cognitive, and/or environmental needs
  • Patient's difficulty with activities of daily living is evaluated in order to provide holistic recommendations resulting in the patient becoming more independent
  • OT can optimize the instrumental activities of daily living such as: power mobility for community access, cognitive strategies for independent living, and home accessibility.


Eligibility
  • Assessment must occur in the home environment
  • Patient must be a resident of British Columbia
  • Patient must be a Canadian citizen, or have permanent resident status
  • Patient requires care following discharge from a hospital, care at home rather than hospitalization, or care because of a terminal illness
How to Access
  • No referral is needed, though a primary healthcare provider can refer their patient to the program
  • If you already have a Case Manager with Home Health and Community Health Services, please contact them directly as they will provide local information tailored to your specific needs
