Physiotherapists and Occupational Therapists work with the patient to restore, improve, or maintain physical independence and safety.
Community Rehabilitation Services provide time-limited, goal-focused physiotherapy and occupational therapy for patients whose mobility, function, cognition, recovery needs, or home environment affect safety and independence at home or in the community.
Physiotherapy and Occupational Therapy may be appropriate for patients who require assessment or intervention related to:
- Mobility, transfers, gait, balance, strength, endurance, or functional decline
- Post-acute, post-surgical, or illness-related recovery where home or community-based rehabilitation is required
- Falls risk screening, assessment, and recommendations to reduce risk
- Home safety, accessibility, equipment, or environmental adaptations
- Pain, fatigue, or chronic condition impacts where rehabilitation goals and self-management strategies are appropriate
Eligibility
- Eligibility depends on health conditions, existing supports, and care needs.
- Patient must be a Canadian citizen, or have permanent resident status
- Patient must be a resident of British Columbia
How to Access:
Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, safety, and whether Physiotherapy, Occupational Therapy, or another Community Health Services discipline is appropriate. Services vary by location.
