Provided by Island Health
- Mobility, transfers, gait, balance, strength, endurance, or functional decline
- Post-acute, post-surgical, or illness-related recovery where home or community-based rehabilitation is required
- Falls risk screening, assessment, and recommendations to reduce risk
- Home safety, accessibility, equipment, or environmental adaptations
- Pain, fatigue, or chronic condition impacts where rehabilitation goals and self-management strategies are appropriate
- Eligibility depends on health conditions, existing supports, and care needs.
- Patient must be a Canadian citizen, or have permanent resident status
- Patient must be a resident of British Columbia
Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, safety, and whether Physiotherapy, Occupational Therapy, or another Community Health Services discipline is appropriate. Services vary by location.
