Home Health and Community Care Intake - Shuswap North Okanagan

Interior Health provides a variety of at-home and community services to people with acute, chronic, palliative or rehabilitative health-care needs.
Home Health Services may include:

  • Acquired Brain Injury Services
  • Adult Day Services
  • Choice in supports for independent living (CSIL)
  • End of Life Care / Palliative Care Physicians and clinicians CLICK HERE

  • Home Oxygen Program
  • Home Support Services
  • Medical Assistance in Dying (MAiD)
  • Nutrition and Dietitian Services
  • Wound Care

  • Other services may include:

  • Adult Day Services - Harmony Haven (Salmon Arm): Assists older adults with varying abilities and medical conditions keep their independence. Clients are provided with a day of recreation programming delivered by trained recreation therapy staff. The program’s nurse works with your community care nurse to provide care as needed during the program day, such as wound and catheter care and chronic disease monitoring and education.
  • Caregiver Support: Caregiver support is for people caring for family or friends, because we understand that care giving, even when given with love, can be stressful.
  • Care Management: People with complex health needs requiring extra, ongoing support to live at home safely and independently will benefit from case management as it prevents unnecessary or premature admissions to a hospital or care facility.
  • Health Services for Community Living: a program that provides health services assessment, planning and care supports to youth and adults with developmental disabilities, and their families and caregivers.
  • LifeLine: An easy-to-use personal response service that lets you summon help any time of the day or night.


  • You can access these services for yourself or a family member by contacting a Home & Community Care office in your area or contact a health care provider and/or physician to complete and submit the home health referral form.

    Some services provided free of charge while others include a cost based on your after tax income.

    All referrals for new clients should be faxed to Central Intake at 1-833-656-0252.
